Below is a list of the updates made to the Form 40A with Calculations.

07/09/2008 - The form was updated to fix problem experienced by some users with checkboxes clearing when "Print" button was                         selected.

07/18/2007 - The "Do not use Calculations" option was removed.

05/10/2007 - The form was updated to change font to Courier.

04/12/2007 - The form was changed to correct problem with PTIN field and leading zeroes on ZipCode field.

02/16/2007 - The Form 40A was updated to correct problem with SSN with leading zeroes.

Frequently Asked Questions

• I do not get a copy of my return when I print it out. What is wrong?
  Make sure you are using the green Print this Form button to print your form. If you try to print the form using the Print command in Acrobat or Acrobat Reader, you will get one page with a statement "Please, use the PRINT button on the Form to Print this form. Thank You." and one blank page.

• When I print the Form 40A it cuts off the bottom of the page?
  When you press the Print this Form button, a print module will appear. Change the Page Scaling to "Fit to Paper".
