Updates

04/08/2009 - The PPT was updated to correct the validation on the beginning and ending determination dates.

06/10/2008 - The PPT was updated to add a button to link to a site to find the NAICS codes.

03/17/2008 - The PPT was updated to fix error with validation of Beginning and Ending Determination Dates.

03/14/2008 - The PPT was updated to allow the correct a problem with entering a valid Date.

02/06/2008 - The PPT was updated to allow the PTIN to be entered in the Preparer's Social Security Number field. Errors with entering the State and Preparer's Sign Date were corrected.

 

Frequently Asked Questions

• I do not get a copy of my return when I print it out. What is wrong?
  Make sure you are using the green Print this Form button to print your form. If you try to print the form using the Print command in Acrobat or Acrobat Reader, you will get one page with a statement "Please, use the PRINT button on the Form to Print this form. Thank You." and eight blank pages.

• I only need the Form PPT and Schedule AL-CAR. Do I have to print all the pages?
  No. When you press the Print this Form button, a print module will appear. You can change the Print Range to select the pages that are needed.

• When I print the Form PPT it cuts off the bottom of the page?
  When you press the Print this Form button, a print module will appear. Change the Page Scaling to "Fit to Paper".
