1. Must be operable and verifiable land-line – NO CELLULAR TELEPHONES
   
   
2. Must be listed with the telephone company in the name of the business as shown on the application.  If the business is an individual, the phone may be listed in the individual’s name otherwise it must be listed in the name of the business.
   
   
3. Must be for the location(s) listed on the application.
   
   
4. Proof of telephone may include a copy of a recent bill (current or previous billing cycle) or a statement from your telephone company which includes contact information for the company representative.
   

Note:  Telephone service is subject to future verification.  If the telephone number is no longer in service, the regulatory license is subject to revocation.
